Oakville North-Burlington – May 8, 2026 – More than 10,000 residents across Oakville, Halton Hills, and Milton who do not currently have a family doctor will soon have new opportunities to connect to primary care, following a $4.1 million provincial investment in local, team-based health care.
The funding announced today as part of Ontario’s Primary Care Action Plan, will support the expansion of Interprofessional Primary Care Teams (IPCTs) across the Halton community. These teams bring together family doctors, nurse practitioners, nurses, social workers, and other health professionals to work collaboratively so patients can get the right care, sooner and closer to home.
The initiative is being coordinated through the Connected Care Halton Ontario Health Team (CCHOHT) in partnership with local primary care and community organizations.
What This Means for Residents
This investment will help people who currently do not have a primary care provider by:
• Expanding team-based primary care services in the community
• Making it easier to navigate health and community supports
• Improving coordination and continuity of care over time
• Helping reduce emergency department visits
“Through a $4.1 million investment, our government is taking meaningful action to connect more than 10,000 residents across Oakville, Halton Hills, and Milton to the primary care they need,” said Effie Triantafilopoulos, MPP for Oakville North-Burlington. “By expanding interprofessional primary care teams, we are ensuring that people can access timely, coordinated care closer to home—whether that’s seeing a family doctor, a nurse practitioner, or other health professionals. This investment is about improving access, reducing pressure on our hospitals, and building a stronger, more connected health care system for families across Halton.
“Our government is putting Halton residents first by bringing primary care directly to our doorsteps. I am incredibly proud that this investment will connect 10,000 more neighbours to the healthcare they deserve, right here in our community,” said Zee Hamid, MPP for Milton
Working Together Across Halton:
Local organizations participating in this initiative include:
• Dorval Medical Associates Family Health Team
• Halton Hills Family Health Team
• OakMed Family Health Team
• Prime Care Family Health Team
• Support House
Led by the CCHOHT, they will bring together patient, family, caregiver, clinical, and administrative voices to ensure the approach reflects community needs. “Navigating the healthcare system alone can be stressful. This is about more than funding—it’s about people being cared for and supported in their own community. Expanding local primary care teams will make a real difference for everyone who has been waiting too long to get the care they need,” said Michele Sparling, Chair Patient, Family, Caregiver Advisory Committee, CCHOHT.
Part of a Broader Provincial Plan
Across Ontario, the province is investing in new and expanded interprofessional primary care teams to connect 500,000 more people to primary care. This work supports the province’s goal of ensuring every Ontarian is connected to primary care by 2029.
Locally, the CCHOHT, Halton Healthcare, and other Halton health leaders have come together and created a shared plan Primary Health Care Action Plan to build a primary care system that is stronger, more connected, and works better for everyone
